This is one of the most misunderstood parts of the category, and one that misleading marketing regularly exploits. Some methods - particularly certain surgical techniques that release the suspensory ligament connecting the penis to the pubic bone - primarily change how the penis hangs when flaccid. They can make the penis appear longer at rest without meaningfully changing erect length.
If erect length or girth is your actual goal, it's worth asking any method you're considering directly whether the claimed change applies to the flaccid state, the erect state, or both.
